Job Description

Indiana University Health is seeking a compassionate and dedicated Entry-Level Patient Navigator to join our team in Fishers. This vital role focuses on providing essential healthcare support, guiding patients through complex healthcare systems, and ensuring they receive timely and appropriate care. You will be the primary point of contact for patients, offering support, information, and advocacy to help them navigate their healthcare journey with confidence and ease. This is an excellent opportunity for individuals new to the healthcare field who possess strong interpersonal skills and a desire to serve.
Key Responsibilities
- Serve as a primary point of contact and resource for patients, guiding them through various stages of their healthcare journey.
- Assist patients in scheduling appointments, understanding medical procedures, and accessing necessary healthcare services.
- Coordinate communication between patients, families, and healthcare providers to ensure seamless care delivery.
- Provide emotional support, active listening, and advocacy for patients facing health challenges.
- Educate patients on their treatment plans, medication adherence, and available community resources.
- Help patients overcome barriers to care, such as transportation, financial concerns, or language differences.
- Maintain accurate and confidential patient records in accordance with privacy regulations.
- Collaborate with multidisciplinary teams to optimize patient outcomes and satisfaction.
