Job Description

We are seeking a compassionate and highly skilled Gerontology Nurse Practitioner – Senior Care Specialist to join our dedicated team in San Francisco. In this vital role, you will provide comprehensive primary and specialized care to our senior patient population, focusing on promoting health, preventing illness, and managing chronic conditions inherent to aging. You will work autonomously and in collaboration with physicians and other healthcare professionals to develop and implement individualized care plans, ensuring the highest quality of life for our elderly members. This position requires a deep understanding of geriatric syndromes, polypharmacy management, palliative care principles, and strong patient and family education skills.

Key Responsibilities

  • Conduct comprehensive geriatric assessments, including physical exams, health histories, and cognitive screenings.
  • Diagnose and treat acute and chronic health conditions common in older adults, such as dementia, heart failure, diabetes, and mobility impairments.
  • Develop and implement individualized care plans, including medication management, lifestyle modifications, and referrals to specialists or community resources.
  • Provide extensive patient and family education on disease management, health promotion, and advance care planning.
  • Monitor patient progress, adjust treatment plans as necessary, and ensure continuity of care across various settings.
  • Collaborate effectively with interdisciplinary teams, including physicians, social workers, physical therapists, and dietitians.
  • Participate in quality improvement initiatives and contribute to the development of best practices in geriatric care.
  • Maintain accurate and thorough electronic health records.

Required Skills

  • Current California Registered Nurse (RN) license.
  • Current California Nurse Practitioner (NP) license.
  • Certification as a Gerontology Nurse Practitioner (GNP) or Adult-Gerontology Nurse Practitioner (AGNP).
  • Master's or Doctorate degree from an accredited Nurse Practitioner program.
  • Minimum of 2 years of experience as a Nurse Practitioner, preferably in geriatrics or internal medicine.
  • Proficient in conducting comprehensive geriatric assessments and managing complex chronic conditions.
  • Excellent communication, interpersonal, and empathy skills.
  • Ability to work autonomously and as part of a collaborative healthcare team.
  • Strong knowledge of polypharmacy management and its implications in the elderly.

Preferred Qualifications

  • Experience within a large integrated healthcare system.
  • Bilingual proficiency (e.g., Spanish, Cantonese, Mandarin) is a plus.
  • Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).
  • Experience with electronic health record systems, particularly Epic.
