Job Description

We are seeking a highly motivated and experienced Registered Nurse (RN) Case Manager to join our remote team. This is a unique opportunity to work from anywhere in the USA, providing comprehensive case management services to a diverse patient population. As an RN Case Manager, you will be instrumental in coordinating care, advocating for patients, and ensuring optimal health outcomes by guiding them through complex healthcare systems. You will collaborate with patients, families, and healthcare providers to develop and implement individualized care plans, focusing on quality, cost-effectiveness, and patient satisfaction. This role requires excellent communication, critical thinking, and organizational skills, along with a passion for patient advocacy.

Key Responsibilities

  • Conduct comprehensive assessments of patient needs, including physical, psychological, social, and financial factors.
  • Develop, implement, and monitor individualized care plans in collaboration with patients, families, and healthcare teams.
  • Coordinate patient care across various settings, including acute care, post-acute care, and home health services.
  • Educate patients and families on their health conditions, treatment options, and self-management strategies.
  • Facilitate communication and collaboration among all members of the healthcare team.
  • Identify and address barriers to care, including social determinants of health.
  • Monitor patient progress and revise care plans as necessary to ensure optimal outcomes.
  • Advocate for patients' rights and needs, ensuring they receive appropriate and timely care.
  • Maintain accurate and thorough documentation of all case management activities in compliance with regulatory standards.
  • Participate in interdisciplinary team meetings and contribute to program development and quality improvement initiatives.

Required Skills

  • Active, unrestricted Registered Nurse (RN) license in one of the 50 US states.
  • Minimum of 3 years of clinical nursing experience.
  • Proven experience in case management, care coordination, or utilization management.
  • Strong understanding of healthcare systems, including managed care and community resources.
  • Excellent written and verbal communication skills.
  • Proficiency in using electronic health records (EHR) and other healthcare technology platforms.
  • Ability to work independently and manage a diverse caseload effectively.
  • Strong critical thinking, problem-solving, and decision-making abilities.
  • Empathy and strong patient advocacy skills.
  • High-speed internet access and a dedicated home office space suitable for remote work.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) degree.
  • Certified Case Manager (CCM) certification or other relevant certifications (e.g., ACM).
  • Experience with telephonic case management.
  • Familiarity with Medicare, Medicaid, and commercial insurance guidelines.
