Job Description

Key Responsibilities
- Assist patients with activities of daily living (ADLs) including bathing, dressing, grooming, and feeding.
- Provide light housekeeping tasks, such as making beds, tidying patient areas, and laundry.
- Assist with mobility and transfers, including turning and positioning patients to prevent bedsores.
- Monitor and report changes in patient condition to the supervising nurse or case manager.
- Prepare meals according to dietary restrictions and assist with feeding as needed.
- Take and record vital signs as instructed by the supervising nurse.
- Provide companionship and emotional support to patients.
- Ensure a safe and clean environment for patients.
- Assist with prescribed exercises and ambulation under the direction of a therapist.
- Document care provided accurately and in a timely manner.
Required Skills
- High School Diploma or GED
- Valid Home Health Aide (HHA) certification in California
- Current CPR certification
- Ability to read, write, and communicate effectively in English
- Strong interpersonal and communication skills
- Empathy and compassion for patients
- Ability to work independently and manage time effectively
- Reliable transportation and valid driver's license with proof of insurance
Preferred Qualifications
- Previous experience in a home health setting
- Experience with electronic health records (EHR)
- Bilingual skills (especially Spanish)
