Job Description

Key Responsibilities
- Provide personal care, including bathing, grooming, dressing, and hygiene assistance, in accordance with the patient's care plan.
- Assist patients with mobility and transfers, including ambulation, using appropriate equipment.
- Administer medication reminders as directed by the supervising nurse.
- Monitor and report changes in patient condition to the supervising nurse promptly.
- Assist with light housekeeping tasks directly related to the patient's immediate environment.
- Provide companionship and emotional support to patients, fostering a positive and comfortable atmosphere.
- Assist with meal preparation and feeding as needed, ensuring nutritional needs are met.
- Document care provided accurately and in a timely manner.
- Adhere to all health and safety regulations and Sutter Health policies and procedures.
- Communicate effectively with patients, family members, and the healthcare team.
Required Skills
- Current Certified Home Health Aide (CHHA) certification in California.
- Demonstrated empathy, compassion, and patience in patient care.
- Excellent interpersonal and communication skills.
- Ability to follow written and verbal instructions precisely.
- Reliability and strong organizational skills.
- Ability to work independently and manage time effectively.
- Physical ability to perform tasks such as lifting, bending, and standing for extended periods.
- Valid driver's license and reliable transportation.
Preferred Qualifications
- Minimum of 1 year of experience as a Home Health Aide in a professional setting.
- Experience with electronic health records (EHR) systems.
- CPR and First Aid certification.
