Job Description

Are you passionate about making a tangible difference in the lives of young people? Scope Australia is seeking an enthusiastic and compassionate 18 Year Old Teens Social Support Worker to join our vibrant team in Melbourne. This role is focused on providing personalised support to young individuals accessing the National Disability Insurance Scheme (NDIS), helping them to develop essential life skills, engage in community activities, and achieve their personal goals. You will be a key facilitator in fostering independence, social connection, and overall well-being for the teens you support. This is an exciting opportunity for someone dedicated to community development and empowering young people.

Key Responsibilities

  • Provide individualised support to NDIS participants, specifically 18-year-old teens, to meet their personal goals and NDIS plan objectives.
  • Assist young people in developing life skills, including budgeting, meal preparation, public transport use, and household management.
  • Facilitate and support participation in community activities, social groups, educational pursuits, and recreational opportunities.
  • Develop and implement engaging support plans tailored to the interests and needs of each young person.
  • Maintain accurate and timely records of support provided, progress notes, and incident reports.
  • Collaborate effectively with families, carers, and other service providers to ensure a coordinated approach to support.
  • Advocate for the rights and choices of young people, promoting their independence and self-determination.
  • Ensure a safe and inclusive environment, adhering to all organisational policies and NDIS quality standards.

Required Skills

  • Strong interpersonal and communication skills, with an ability to build rapport with young people.
  • Empathy, patience, and a genuine desire to support individuals with disability.
  • Ability to work independently and as part of a multidisciplinary team.
  • Effective problem-solving skills and a proactive approach to challenges.
  • Current Australian Driver's License and access to a reliable vehicle.
  • Current Working With Children Check (WWCC) and NDIS Worker Screening Check.
  • Basic computer literacy for record-keeping and communication.

Preferred Qualifications

  • Certificate III or IV in Disability, Community Services, Youth Work, or a related field.
  • Experience working with young people or individuals with disability, particularly those within the NDIS framework.
  • Current First Aid and CPR certificates.
