Job Description
We are seeking a compassionate and highly skilled Gerontology Nurse Practitioner – Senior Care Specialist to join our dedicated team in Berkeley, CA. In this vital role, you will provide comprehensive primary and specialty care services to our senior patient population, focusing on promoting health, managing chronic conditions, and enhancing the quality of life for older adults. You will collaborate closely with an interdisciplinary team to develop and implement individualized care plans, ensuring holistic and patient-centered care.
Key Responsibilities
- Conduct comprehensive geriatric assessments, including physical, cognitive, functional, and psychosocial evaluations.
- Diagnose and treat acute and chronic health conditions common in older adults.
- Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and the healthcare team.
- Prescribe medications and other therapies as appropriate, adhering to established protocols and state regulations.
- Provide health education and counseling to patients and their families on disease prevention, health promotion, and self-management strategies.
- Perform routine health screenings and preventative care for geriatric patients.
- Refer patients to specialists or other healthcare providers as needed, ensuring continuity of care.
- Participate in team meetings, case conferences, and quality improvement initiatives.
- Maintain accurate and thorough electronic health records.
Required Skills
- Current, unencumbered California Registered Nurse (RN) license.
- Current, unencumbered California Nurse Practitioner (NP) license with Gerontology or Adult-Gerontology certification.
- Master's degree or higher in Nursing from an accredited program.
- DEA registration.
- Minimum of 3 years of clinical experience as a Nurse Practitioner, preferably in geriatrics or adult primary care.
- Strong knowledge of common geriatric syndromes, polypharmacy, and age-related physiological changes.
- Excellent communication, interpersonal, and empathy skills.
- Ability to work effectively in an interdisciplinary team environment.
- Proficiency in electronic health record (EHR) systems.
Preferred Qualifications
- Board Certification in Gerontology or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP-BC).
- Experience with integrated healthcare delivery systems.
- Bilingual proficiency (e.g., Spanish, Cantonese, Mandarin).
