Job Description
We are seeking a dedicated and detail-oriented Medication History Technician to join our hospital-based team. In this role, you will play a crucial part in ensuring patient safety by accurately obtaining and documenting comprehensive medication histories for patients admitted to our facility. This position offers significant clinical exposure, working directly with patients, pharmacists, nurses, and physicians to gather critical information. You will be responsible for interviewing patients and/or their families, reviewing external medical records, and collaborating with the care team to create an accurate and complete medication profile upon admission. The ideal candidate will possess excellent communication skills, a meticulous approach to data collection, and a strong commitment to patient care.
Key Responsibilities
- Conduct thorough patient and/or family interviews to obtain accurate and complete medication histories, including prescription, over-the-counter, herbal, and supplement use.
- Verify medication information using multiple sources, such as pharmacies, physician offices, and external medical records.
- Document medication histories precisely into the electronic health record (EHR) system within established timeframes.
- Collaborate effectively with pharmacists, nurses, and physicians to clarify discrepancies and ensure medication reconciliation accuracy.
- Communicate effectively with patients and families, explaining the importance of accurate medication history and addressing any concerns.
- Identify and report potential medication-related issues to the clinical care team.
- Maintain patient confidentiality and adhere to all hospital policies and procedures, including HIPAA regulations.
- Participate in ongoing training and educational opportunities to enhance knowledge and skills related to medication safety and patient care.
Required Skills
- High School Diploma or GED
- Excellent verbal and written communication skills
- Strong attention to detail and accuracy
- Proficiency in basic computer applications and data entry
- Ability to work independently and as part of a multidisciplinary team
- Demonstrated ability to interact compassionately and effectively with patients and families
- Basic medical terminology knowledge
Preferred Qualifications
- Associate's or Bachelor's degree in a healthcare-related field
- Certified Pharmacy Technician (CPhT) certification
- Previous experience in a healthcare setting, particularly in patient care or pharmacy
- Familiarity with electronic health record (EHR) systems
