Job Description
We are seeking a detail-oriented and meticulous Medication Reconciliation Technician with a strong focus on clinical data to join our team in Moreno Valley, CA. In this vital role, you will play a crucial part in patient safety and quality care by accurately collecting, verifying, and documenting patient medication histories. Your expertise in clinical data will ensure the integrity and completeness of electronic health records, supporting physicians and pharmacists in making informed treatment decisions. This position requires excellent communication skills, a keen eye for detail, and a commitment to maintaining precise patient information.
Key Responsibilities
- Obtain and document comprehensive medication histories for patients by interviewing patients, family members, caregivers, and utilizing all available resources, including pharmacy records and primary care provider offices.
- Reconcile medication lists during patient admissions, transfers, and discharges, identifying and resolving discrepancies to prevent medication errors.
- Verify medication names, dosages, routes, frequencies, and last dose taken, ensuring accuracy and completeness of all data entered into the electronic health record (EHR).
- Collaborate effectively with pharmacists, physicians, nurses, and other healthcare professionals to clarify medication information and resolve discrepancies.
- Educate patients and families on the importance of an accurate medication list and provide instructions on how to maintain one.
- Maintain strict confidentiality of patient information in accordance with HIPAA regulations.
- Participate in quality improvement initiatives related to medication reconciliation processes and clinical data integrity.
Required Skills
- High School Diploma or GED required.
- Current Certified Pharmacy Technician (CPhT) certification (or equivalent state licensure/certification if CPhT is not applicable in California) required.
- Minimum of 2 years of experience as a Pharmacy Technician, preferably in a hospital or clinical setting.
- Proficiency in medical terminology, particularly related to pharmacology and medication classes.
- Demonstrated strong attention to detail and accuracy in data entry and verification.
- Excellent verbal and written communication skills to interact with patients, families, and healthcare professionals.
- Ability to work independently and collaboratively in a fast-paced clinical environment.
- Proficiency with electronic health record (EHR) systems; EPIC experience highly desirable.
Preferred Qualifications
- Associate's or Bachelor's degree in a healthcare-related field.
- Previous experience specifically performing medication reconciliation.
- Familiarity with clinical decision support systems and pharmacy information systems.
- Bilingual skills (English/Spanish) highly desirable due to patient population.
