Job Description
We are seeking a highly motivated and experienced Registered Nurse to join our team as a Transplant Coordinator. This critical role involves managing the complex journey of transplant patients, from initial evaluation through post-transplant care. The successful candidate will demonstrate exceptional organizational skills, clinical expertise, and a compassionate approach to patient care. Key Responsibilities
- Coordinate all aspects of the transplant process for assigned patients, including pre-transplant evaluation, listing, and post-transplant follow-up.
- Serve as the primary liaison for patients, their families, and the multidisciplinary transplant team (physicians, surgeons, social workers, dietitians, pharmacists).
- Provide comprehensive education to patients and families regarding transplant procedures, medication regimens, lifestyle adjustments, and potential complications.
- Facilitate and track all necessary diagnostic testing and consultations for pre-transplant evaluations.
- Manage patient waitlist status, ensuring compliance with UNOS (United Network for Organ Sharing) guidelines and timely allocation processes.
- Monitor patient progress post-transplant, coordinating follow-up appointments, medication management, and addressing emerging clinical needs.
- Maintain accurate, detailed, and up-to-date electronic health records for all transplant patients.
- Participate in transplant program meetings, quality improvement initiatives, and ongoing program development.
- Advocate for patients, ensuring they receive the highest quality of care and support throughout their transplant journey.
Required Skills
- Current Registered Nurse (RN) license in the Commonwealth of Virginia.
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Minimum of 3-5 years of recent clinical nursing experience, preferably in critical care, nephrology, oncology, or a transplant-related field.
- Demonstrated ability to work autonomously and manage a complex patient caseload effectively.
- Excellent verbal and written communication skills, with the ability to convey complex medical information clearly.
- Strong organizational skills and attention to detail.
- Proficiency in using electronic health records (EHR) systems.
- Compassionate and patient-centered approach to care.
Preferred Qualifications
- Certified Clinical Transplant Coordinator (CCTC) certification.
- Master of Science in Nursing (MSN).
- Prior experience working within an organ transplant program or an Organ Procurement Organization (OPO).
- Bilingual proficiency (e.g., Spanish) is a plus.
