Job Description
We are seeking a dedicated and compassionate Community Health Aide with an outreach focus to join our dynamic team. This full-time position operates Monday to Friday and is crucial in supporting our community health initiatives by connecting with individuals and families directly in the community. The successful candidate will assist with health promotion, provide basic health education, facilitate access to resources, and support clients in navigating the healthcare system. This role involves significant field work and requires excellent interpersonal skills and a commitment to serving diverse populations.
Key Responsibilities
- Conduct outreach activities in designated community areas to identify individuals in need of health services and support.
- Provide basic health education on topics such as disease prevention, healthy living, and chronic disease management.
- Assist clients with navigating the healthcare system, including scheduling appointments and understanding services.
- Distribute health information materials and resources.
- Record and maintain accurate client data and outreach activity logs.
- Collaborate with community partners and healthcare professionals to ensure coordinated care.
- Identify and report community health trends and needs to supervisors.
- Participate in team meetings and professional development opportunities.
Required Skills
- High School Diploma or equivalent.
- Minimum 1 year of experience in a community outreach or healthcare support role.
- Strong interpersonal and communication skills (verbal and written).
- Ability to work independently and as part of a multidisciplinary team.
- Valid driver's license and access to a reliable vehicle for community travel.
- Demonstrated cultural sensitivity and ability to work with diverse populations.
- Basic computer proficiency for data entry and record keeping.
Preferred Qualifications
- Certificate or Diploma in Community Health, Social Work, or a related field.
- Experience working with vulnerable populations (e.g., seniors, homeless, newcomers).
- Knowledge of local community resources and services in Toronto.
- Proficiency in languages other than English commonly spoken in Toronto communities (e.g., Mandarin, Tagalog, Punjabi, Urdu, Spanish).
