Job Description
We are seeking a highly motivated and skilled Family Nurse Practitioner (FNP) to join a thriving private practice within the PIH Health network in South Gate, CA. This is an exceptional opportunity for an FNP looking to provide comprehensive primary care services in an autonomous yet supported setting, focusing on long-term patient relationships and continuity of care. The successful candidate will manage a diverse patient panel, performing health assessments, diagnosing and treating acute and chronic conditions, prescribing medications, and providing health education and counseling to individuals and families across the lifespan. This role emphasizes a high standard of clinical excellence and patient satisfaction.
Key Responsibilities
- Provide comprehensive primary care services to patients of all ages, including infants, children, adolescents, adults, and geriatrics.
- Conduct thorough medical histories, physical examinations, and diagnostic tests.
- Diagnose and treat acute illnesses and chronic health conditions.
- Order, interpret, and evaluate diagnostic tests.
- Prescribe medications and other treatments as appropriate.
- Develop and implement individualized patient care plans.
- Provide preventive health maintenance, screenings, and immunizations.
- Educate patients and families on health management, disease prevention, and treatment options.
- Collaborate with physicians and other healthcare professionals to ensure coordinated patient care.
- Maintain accurate and confidential patient records.
- Participate in quality improvement initiatives and continuing education.
- Refer patients to specialists as needed.
Required Skills
- Current, unencumbered California Registered Nurse (RN) license.
- Current, unencumbered California Nurse Practitioner (NP) license.
- Current national certification as a Family Nurse Practitioner (FNP) from an accredited board (e.g., AANP, ANCC).
- Current DEA registration.
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Strong clinical assessment and diagnostic skills.
- Proficiency in electronic health records (EHR) systems.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and as part of a multidisciplinary team.
Preferred Qualifications
- Master's or Doctorate degree in Nursing from an accredited program.
- Bilingual proficiency in Spanish is highly desirable.
- Experience in a private practice or outpatient clinic setting.
- Experience with PIH Health's specific EHR system (e.g., Epic).
