Job Description
We are seeking a dedicated and detail-oriented Medication History Technician to join our hospital-based team in New Port Richey, Florida. This critical role involves obtaining accurate and comprehensive medication histories from patients upon admission or during their hospital stay. The ideal candidate will have strong communication skills, a meticulous approach to data collection, and a commitment to patient safety. This position offers significant clinical exposure, working directly with patients, pharmacists, nurses, and physicians to ensure the most accurate medication information is available for patient care decisions. You will play a vital role in preventing medication errors and optimizing patient outcomes.
Key Responsibilities
- Obtain and document comprehensive medication histories from patients, families, and external healthcare providers (e.g., retail pharmacies, physician offices) within established timeframes.
- Clarify discrepancies or missing information by contacting patients' pharmacies, physicians, and other healthcare facilities.
- Input accurate medication history data into the electronic health record (EHR) system.
- Collaborate effectively with pharmacists, nurses, and medical staff to ensure complete and accurate medication reconciliation.
- Educate patients and families on the importance of an accurate medication list.
- Maintain confidentiality of patient information at all times.
- Participate in ongoing training and education pertinent to medication safety and patient care.
- Adhere to all hospital policies, procedures, and regulatory requirements.
Required Skills
- High School Diploma or GED
- Active Pharmacy Technician certification (PTCB or NPTA) or state registration as required
- Minimum 6 months of experience as a Pharmacy Technician, preferably in a hospital or retail setting
- Excellent verbal communication and interpersonal skills
- Strong attention to detail and accuracy
- Proficiency in basic computer skills and electronic health record systems
- Ability to work effectively in a fast-paced, clinical environment
- Knowledge of medical terminology and common medication classes
Preferred Qualifications
- Associate's or Bachelor's degree in a healthcare-related field
- Previous experience specifically in medication history or medication reconciliation
- Familiarity with AdventHealth's EHR system (e.g., Cerner, Epic)
- Bilingual skills (e.g., Spanish/English)
