Job Description
We are seeking a dedicated and compassionate Diabetic Educator Nurse to join our patient support team. This vital role involves educating patients with diabetes and their families about disease management, self-care techniques, medication adherence, and lifestyle modifications. You will empower patients to take control of their health through personalized education and ongoing support. While this is a full-time position, we offer flexible scheduling options to ensure a healthy work-life balance for our team members. This is an on-site role, providing direct patient interaction in our Downtown Lincoln facility.
Key Responsibilities
- Provide individualized and group education to patients newly diagnosed with diabetes or those requiring advanced management strategies.
- Assess patient learning needs, readiness to learn, and current knowledge regarding diabetes self-management.
- Develop and implement personalized education plans covering topics such as glucose monitoring, medication administration (including insulin), nutrition, exercise, and complication prevention.
- Monitor and evaluate patient progress, adjusting education plans as necessary to achieve optimal health outcomes.
- Collaborate with physicians, dietitians, and other healthcare professionals to ensure comprehensive patient care.
- Document all patient education and care activities accurately and timely in the electronic health record.
- Stay current with the latest evidence-based practices and advancements in diabetes care and education.
- Act as a resource for other healthcare staff regarding diabetes management.
- Provide emotional support and motivation to patients and their families.
- Participate in community health initiatives and outreach programs related to diabetes awareness and prevention.
Required Skills
- Current Registered Nurse (RN) license in the State of Nebraska.
- Minimum of 3 years of clinical nursing experience, with at least 1 year in diabetes care.
- Strong understanding of diabetes pathophysiology, pharmacology, and management principles.
- Excellent communication, interpersonal, and teaching skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in using electronic health records (EHR) systems.
- Demonstrated empathy and compassion for patients.
Preferred Qualifications
- Certified Diabetes Care and Education Specialist (CDCES) credential.
- Bachelor of Science in Nursing (BSN) degree.
- Experience with various diabetes technologies (e.g., insulin pumps, continuous glucose monitors).
- Bilingual proficiency (especially Spanish) is a plus.
- Experience in an outpatient clinic or health education setting.
