Job Description
We are seeking a highly motivated and experienced Registered Nurse to join our renowned Transplant Team as a Transplant Coordinator. This critical role involves managing and coordinating all aspects of the transplant process, from evaluation to post-transplant care, for patients undergoing organ transplantation. The successful candidate will serve as a central point of contact for patients, families, and multidisciplinary healthcare teams, ensuring seamless communication, education, and support throughout the complex transplant journey. This is a high-responsibility position demanding exceptional clinical judgment, organizational skills, and a deep commitment to patient advocacy.
Key Responsibilities
- Coordinate the pre-transplant evaluation process, including patient screening, diagnostic testing, and multidisciplinary consultations.
- Educate patients and their families about the transplant process, potential risks, benefits, and lifestyle changes.
- Manage patient waitlist status, communicate updates, and prepare patients for transplantation.
- Facilitate donor organ offers and coordinate rapid patient notification and admission for transplant surgery.
- Provide comprehensive post-transplant care coordination, including medication management, follow-up appointments, and symptom monitoring.
- Act as a liaison between patients, physicians, social workers, pharmacists, and other healthcare professionals.
- Maintain accurate and timely documentation in patient records and transplant databases.
- Participate in ongoing quality improvement initiatives and transplant program development.
- Ensure compliance with UNOS (United Network for Organ Sharing) regulations and institutional policies.
Required Skills
- Active Registered Nurse (RN) license in the state of Kansas
- Bachelor of Science in Nursing (BSN)
- Minimum of 4 years of clinical nursing experience, preferably in critical care, medical-surgical, or specialty areas such as nephrology or cardiology
- Exceptional communication and interpersonal skills, with the ability to empathize and build rapport with patients and families
- Strong organizational skills and ability to manage multiple complex cases simultaneously
- Proficiency in electronic health records (EHR) systems
- Demonstrated ability to work effectively within a multidisciplinary team
- Critical thinking and problem-solving abilities under pressure
Preferred Qualifications
- Certified Clinical Transplant Coordinator (CCTC) certification or eligibility
- Previous experience in a transplant program or related specialty area
- Master's degree in Nursing or related field
- Knowledge of UNOS regulations and transplant guidelines
- Experience with patient education and counseling
