Job Description

Key Responsibilities
- Provide personal care to hospice patients, including bathing, dressing, grooming, and feeding.
- Assist patients with mobility, transfers, and exercises as directed by the nursing care plan.
- Monitor and report changes in the patient's condition to the supervising registered nurse.
- Perform light housekeeping duties related to the patient's immediate environment.
- Offer emotional support and companionship to patients and their families.
- Accurately document services provided and patient observations.
- Adhere to all safety protocols and infection control procedures.
- Communicate effectively with patients, families, and the interdisciplinary hospice team.
Required Skills
- Current Certified Nursing Assistant (CNA) certification or Home Health Aide (HHA) certification in California.
- High school diploma or equivalent.
- Minimum of 1 year of experience in a healthcare setting, preferably in hospice, home health, or acute care.
- Proven ability to provide compassionate and patient-centered care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid driver's license, reliable transportation, and auto insurance.
Preferred Qualifications
- Experience specifically in hospice or palliative care.
- Bilingual skills (especially Spanish) are a plus.
- Knowledge of end-of-life care principles and practices.
