Job Description
We are seeking a dedicated and compassionate Gerontology Nurse Practitioner to join our senior care team in Aurora, Colorado. The ideal candidate will provide specialized primary and acute care to our elderly patient population, focusing on promoting health, managing chronic conditions, and enhancing quality of life. This role involves working collaboratively with a multidisciplinary team to deliver holistic, patient-centered care within an inpatient or outpatient setting.
Key Responsibilities
- Conduct comprehensive geriatric assessments, including physical exams, medical history reviews, and functional status evaluations.
- Diagnose and manage acute and chronic health conditions common in older adults, such as diabetes, heart disease, dementia, and mobility impairments.
- Develop and implement individualized care plans, including prescribing medications, ordering diagnostic tests, and initiating appropriate treatments.
- Provide patient and family education on disease management, health promotion, and preventive care strategies.
- Collaborate with physicians, registered nurses, social workers, therapists, and other healthcare professionals to ensure coordinated and integrated care.
- Perform routine health screenings and immunizations as per established guidelines.
- Participate in regular team meetings, case conferences, and quality improvement initiatives.
- Maintain accurate and thorough electronic health records.
Required Skills
- Current Advanced Practice Registered Nurse (APRN) license in Colorado.
- National Board Certification as a Gerontological Nurse Practitioner (GNP) or Adult-Gerontology Nurse Practitioner (AGNP).
- Master's or Doctoral degree in Nursing from an accredited program.
- DEA registration and prescriptive authority.
- Strong clinical assessment, diagnostic, and treatment planning skills specific to the geriatric population.
- Excellent communication, interpersonal, and empathy skills.
- Ability to work autonomously and as part of a collaborative team.
- Proficiency in electronic health record (EHR) systems.
Preferred Qualifications
- Minimum of 2 years of experience as a Nurse Practitioner, preferably in geriatrics or senior care.
- Experience with palliative care or hospice services.
- Bilingual proficiency (e.g., Spanish) is a plus.
- Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).
