Job Description

This Medication Reconciliation Technician position at Banner Health in Gilbert, AZ, offers a unique focus on the meticulous management and analysis of clinical data related to patient medications. The role involves accurately reconciling patient medication lists across various healthcare settings and systems, ensuring data integrity, and contributing significantly to patient safety initiatives. The successful candidate will leverage strong analytical skills to identify discrepancies, collaborate effectively with healthcare providers, and maintain precise medication records within our electronic health record (EHR) system. Key Responsibilities

  • Perform comprehensive medication reconciliation for patients upon admission, transfer, and discharge, with a keen eye on clinical data accuracy.
  • Verify the accuracy of medication lists by comparing information from multiple sources, including patient interviews, pharmacy records, and external health systems.
  • Identify and resolve discrepancies in medication orders and administration records, escalating complex issues to pharmacists or prescribing providers promptly.
  • Maintain and update patient medication profiles within the electronic health record (EHR) system, ensuring data integrity and completeness.
  • Collaborate effectively with pharmacists, physicians, nurses, and other healthcare professionals to ensure seamless and safe medication management.
  • Participate actively in quality improvement initiatives related to medication safety, data quality, and process enhancements.
  • Educate patients and their families regarding medication regimens, potential side effects, and adherence, as appropriate.
  • Assist in the development and refinement of medication reconciliation protocols and procedures, with an emphasis on data standardization.
  • Generate and analyze reports on medication reconciliation metrics and clinical data trends to support operational insights and improvements.

Required Skills

  • Certified Pharmacy Technician (CPhT) certification is mandatory.
  • Minimum of 1 year of experience in a pharmacy setting or a related healthcare environment.
  • Proficiency with electronic health record (EHR) systems and pharmacy information systems.
  • Exceptional attention to detail and superior organizational skills.
  • Demonstrated ability to identify, analyze, and resolve medication discrepancies.
  • Excellent communication and interpersonal skills, essential for team collaboration.
  • Solid understanding of pharmaceutical terminology, drug names, dosages, and administration routes.
  • Proficiency in data entry and basic data analysis principles.

Preferred Qualifications

  • Associate's degree in a healthcare-related field or equivalent post-secondary education.
  • Experience with specific EHR systems used by Banner Health (e.g., Cerner, Epic).
  • Previous experience specifically in medication reconciliation roles or clinical data management.
  • Familiarity with clinical data management principles and best practices.
  • Strong analytical and problem-solving abilities applied to complex clinical data scenarios.
