Job Description
We are seeking a dedicated and compassionate Diabetic Educator Nurse to join our patient support team in San Antonio. This role offers flexible hours to accommodate a work-life balance while providing crucial education and support to patients living with diabetes. You will empower individuals with the knowledge and skills necessary to manage their condition effectively, prevent complications, and achieve optimal health outcomes. This is a vital position focused on patient education, self-management techniques, and fostering a supportive environment for individuals and their families.
Key Responsibilities
- Provide individualized and group diabetes self-management education (DSME) to patients and their families.
- Assess patient learning needs, readiness to learn, and current diabetes management practices.
- Develop, implement, and evaluate personalized education plans based on evidence-based guidelines and patient goals.
- Educate patients on medication management, blood glucose monitoring, nutrition, exercise, and complication prevention.
- Collaborate with physicians, dietitians, and other healthcare professionals to ensure comprehensive patient care.
- Document patient encounters, education provided, and progress towards goals accurately and timely.
- Stay current with the latest advancements in diabetes treatment and education.
- Participate in quality improvement initiatives related to diabetes care and education.
- Offer empathetic support and guidance, addressing patient concerns and emotional well-being related to diabetes.
Required Skills
- Active Registered Nurse (RN) license in Texas.
- Minimum of 2 years of clinical nursing experience.
- Demonstrated experience in patient education, particularly in chronic disease management.
- Excellent communication, interpersonal, and presentation skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic health records (EHR) systems.
- Strong organizational skills and attention to detail.
- Compassionate and patient-centered approach.
Preferred Qualifications
- Certified Diabetes Care and Education Specialist (CDCES) credential.
- Bachelor of Science in Nursing (BSN) degree.
- Experience with various diabetes technologies (insulin pumps, continuous glucose monitors).
- Bilingual (English/Spanish) skills.
