Job Description
We are seeking a dedicated and compassionate Gerontology Nurse Practitioner to join our dynamic senior care team. This vital role involves providing comprehensive primary and specialized healthcare services to older adults, focusing on health promotion, disease prevention, and the management of acute and chronic conditions specific to geriatric populations. The successful candidate will work collaboratively within an interdisciplinary team to enhance the quality of life and functional independence of our senior patients.
Key Responsibilities
- Conduct comprehensive geriatric assessments, including physical, cognitive, functional, and psychosocial evaluations.
- Diagnose and manage acute and chronic health conditions common in older adults, such as diabetes, heart disease, dementia, and polypharmacy.
- Develop and implement individualized care plans, focusing on patient and family-centered goals.
- Prescribe medications and other therapies in accordance with established protocols and state regulations.
- Provide health education, counseling, and support to patients and their families regarding age-related changes, preventive care, and disease management.
- Collaborate with physicians, social workers, physical therapists, and other healthcare professionals to ensure integrated and holistic patient care.
- Order and interpret diagnostic tests, making appropriate referrals to specialists when necessary.
- Participate in quality improvement initiatives and contribute to the advancement of geriatric nursing practice.
- Maintain accurate and timely electronic health records.
Required Skills
- Current unrestricted Nurse Practitioner license in North Carolina.
- ANCC or AANP Board Certification as an Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) or Adult-Gerontology Acute Care Nurse Practitioner (AGACNP).
- Master's or Doctorate in Nursing from an accredited program.
- Minimum of 2 years of experience as a Nurse Practitioner, preferably in geriatrics or adult primary care.
- Strong clinical assessment, diagnostic, and treatment planning skills.
- Excellent communication, interpersonal, and team collaboration abilities.
- Proficiency in electronic health records (EHR) systems.
Preferred Qualifications
- Prior experience in an academic medical center or specialized geriatric clinic.
- Experience with palliative care or hospice services.
- Membership in professional organizations related to gerontology or advanced practice nursing.
- Bilingual proficiency (e.g., Spanish) is a plus.
