Job Description
We are seeking a highly experienced and dedicated Transplant Coordinator (RN) to join our esteemed Solid Organ Transplant Program. This is a pivotal role demanding exceptional clinical expertise, organizational skills, and a profound commitment to patient advocacy. The successful candidate will manage complex patient cases, guide patients and families through the intricate transplant process, and ensure seamless coordination of care across multidisciplinary teams. Key Responsibilities
- Manage and coordinate the care of pre-transplant candidates, transplant recipients, and living donors.
- Conduct comprehensive patient assessments, including physical, psychological, and social evaluations.
- Educate patients and families on the transplant process, potential risks, benefits, and post-transplant care requirements.
- Serve as the primary point of contact for patients, families, and referring physicians.
- Coordinate diagnostic testing, consultations, and patient evaluations according to established protocols.
- Facilitate patient listings with the United Network for Organ Sharing (UNOS) and manage waiting list status.
- Participate in multidisciplinary team meetings to present patient cases, develop care plans, and discuss outcomes.
- Ensure compliance with all regulatory requirements, including UNOS, CMS, and state health regulations.
- Provide clinical oversight and support during the organ offer and allocation process.
- Document all aspects of patient care accurately and comprehensively in the electronic health record.
- Participate in quality improvement initiatives and program development.
- Provide on-call coverage for transplant emergencies as scheduled.
Required Skills
- Current California Registered Nurse (RN) license in good standing.
- Minimum of 5 years of recent acute care nursing experience, with at least 2 years in transplant, critical care, or nephrology.
- Bachelor of Science in Nursing (BSN).
- Strong understanding of solid organ transplantation principles and patient management.
- Excellent communication, interpersonal, and patient education skills.
- Proven ability to manage complex cases and prioritize workload effectively.
- Proficiency in electronic health record systems.
- Strong critical thinking and problem-solving abilities.
Preferred Qualifications
- Master's degree in Nursing (MSN) or related field.
- Certified Clinical Transplant Coordinator (CCTC) certification.
- Experience with Epic EHR system.
- Fluency in Spanish or other languages common in the San Diego community.
