Job Description
We are seeking a dedicated and experienced Registered Nurse (RN) Case Manager to join our remote team. In this critical role, you will be responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the care options and services required to meet an individual’s health needs. As an RN Case Manager, you will work collaboratively with patients, their families, and healthcare providers to ensure comprehensive, high-quality, and cost-effective care. This is a 100% remote position, allowing you to work from anywhere in the USA, providing essential services to our members across various states.
Key Responsibilities
- Conduct comprehensive assessments of members' health needs, including physical, psychosocial, and environmental factors.
- Develop individualized care plans in collaboration with members, their families, and interdisciplinary healthcare teams.
- Coordinate and facilitate access to appropriate healthcare services, community resources, and support systems.
- Monitor members' progress towards health goals and modify care plans as needed.
- Educate members and their families on health conditions, treatment options, and self-management strategies.
- Advocate for members' rights and ensure they receive optimal care within their benefit plan.
- Document all assessments, care plans, interventions, and outcomes accurately and in a timely manner.
- Ensure compliance with all federal, state, and local regulations, as well as company policies and procedures.
- Participate in quality improvement initiatives and team meetings.
Required Skills
- Active, unrestricted Registered Nurse (RN) license in at least one US state (and ability to obtain multi-state licensure if required).
- Minimum of 3 years of clinical experience as a Registered Nurse.
- Strong clinical assessment and critical thinking skills.
- Excellent verbal and written communication abilities.
- Proficiency in using electronic medical record (EMR) systems and other healthcare technology.
- Ability to work independently and manage a caseload effectively in a remote environment.
- Strong organizational and time management skills.
- Demonstrated ability to build rapport and trust with diverse populations.
Preferred Qualifications
- Bachelor's Degree in Nursing (BSN).
- Case Management certification (e.g., CCM, CCMC).
- Previous experience in managed care, home health, utilization management, or acute care case management.
- Bilingual English/Spanish skills.
