Job Description
AccentCare is seeking a dedicated and compassionate Registered Nurse (RN) to join our hospice team in the New Port Richey area. This role is focused on providing direct patient care in the comfort and familiarity of their own homes. As a Hospice Nurse, you will be a vital part of our interdisciplinary team, delivering holistic care that addresses the physical, emotional, and spiritual needs of our patients and their families during a sensitive time. Your responsibilities will include pain and symptom management, emotional support, end-of-life care planning, and patient/family education. This position requires regular home visits, and we provide mileage reimbursement to support your travel.
Key Responsibilities
- Conduct thorough initial and ongoing assessments of patient and family needs, developing individualized care plans.
- Administer medications, manage symptoms, and perform skilled nursing interventions in accordance with the plan of care.
- Educate patients and their families on disease progression, medication management, and comfort measures.
- Provide compassionate emotional, spiritual, and psychosocial support to patients and their loved ones.
- Collaborate effectively with physicians, social workers, spiritual counselors, and other team members to coordinate comprehensive care.
- Maintain accurate, timely, and complete documentation of patient care observations, interventions, and outcomes in the electronic health record.
- Ensure compliance with all federal, state, and local regulations, as well as AccentCare's policies and procedures.
- Participate in on-call rotation as required to provide continuous patient support.
- Advocate for patient comfort and dignity, ensuring care is aligned with their wishes and values.
Required Skills
- Current and unrestricted Registered Nurse (RN) license in the State of Florida.
- Minimum of 2 years of clinical nursing experience, preferably in hospice, palliative care, oncology, or medical-surgical settings.
- Valid driver's license, reliable transportation, and current auto insurance.
- Strong clinical assessment, critical thinking, and problem-solving abilities.
- Exceptional interpersonal and communication skills, with a compassionate and empathetic approach to patient care.
- Ability to work autonomously and manage a diverse caseload effectively.
- Proficiency in using electronic health records (EHR) systems and other relevant technology.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN) degree.
- Certified Hospice and Palliative Nurse (CHPN) certification.
- Previous experience in home health or community health nursing.
- Bilingual abilities (e.g., Spanish) to better serve a diverse patient population.
